Sliding Fee Discount Program Policy

At this clinic, No one will be denied access to primary care services due to an inability to pay. Health services will be provided at no charge, or at a reduced charge, to persons unable to pay. There is a discounted / Sliding fee program available. Eligibility for this Program is Based solely on family size and income; and applicable to all individuals and families with annual incomes at or below 200 % of the most current Federal Poverty Guidelines. Please contact our office for more information about the program. The discount applies for in - office services only and not for inpatient, speciality care and outside ancillary services.
